Care coordination

Care that keeps moving.

A referral should lead to an appointment. A request should have someone following it. Your navigator helps connect the people, paperwork and next steps around your care.

Start with your life.

Your first conversation is about what matters to you: the care you need, the changes coming up, how you communicate and the people you want involved. You do not need to arrive with everything organized.

Together, you and your navigator choose a few priorities and turn them into a plan. That might mean finding an adult specialist, following up on equipment, or getting ready for an insurance renewal.

Someone stays with the request.

Care coordination is the work between appointments: finding the right contact, checking what is missing and asking again when something has stalled.

  • Referrals and appointments

Help finding the next provider, preparing for the handoff and following up on what still needs to happen.

  • Refills and equipment

Help contacting the people responsible, gathering requested information and tracking the response.

  • Coverage and services

Help identifying deadlines, organizing paperwork and finding the right agency or plan contact.

A plan you can return to.

Your goals and open requests live in one place. For each request, you can see its current step, who is handling it and the updates recorded by your team.

A navigator cannot guarantee an approval or make another organization move faster. They can make the unfinished work visible and keep following up.

Keep the doctors who know you.

Parea works alongside your existing doctors and services. The pilot focuses on coordination, communication and family support. It does not include a virtual clinic, prescribing or weekly medical visits.

Your doctors make clinical decisions. Your navigator helps you prepare for those conversations and follow through afterward.

Use the app when it helps.

Read your plan, follow requests, message the team or ask Helen for help preparing a note. If an app is not the right way for you to communicate, tell your navigator what works better.

Families can follow the updates you choose to share. You should not have to be the messenger between everyone helping you.
