For partners
A clearer handoff into adult care.
We are speaking with health plans, children’s hospitals and disability services agencies about a focused pilot for young adults whose care crosses too many disconnected systems.
Start with the unfinished work.
Young adults and families often need help carrying a referral, renewal or equipment request across organizational boundaries. Parea’s navigator follows that work in a shared plan and records who is responsible for the next step.
The pilot pairs that coordination with Helen’s assistance for members and supporters, plus caregiver coaching. It is designed to complement existing clinicians and services.
Different partners. A shared purpose.
We want to define a manageable starting cohort and clear working relationships with each partner.
Health plans
Discuss which transition and coordination needs are hardest to address, how families can access the pilot and what evidence would support a future contract.
Hospitals and clinical teams
Explore a referral path for young adults moving to adult care, with clear contacts and follow-up expectations.
Disability services agencies
Connect health-related coordination with the services a person already receives, using permission-based communication.
Learn before making bigger claims.
We propose measuring whether care requests move forward, how long they remain unresolved, whether Helen is useful and whether families feel more confident and supported.
We also need to understand the staff time and cost required to deliver the service well. Reduced hospital use, cost savings and productivity improvements are hypotheses to evaluate, not pilot results we can claim today.
Define the relationship together.
Before enrollment, we need agreement on the cohort, responsibilities, referral process, consent, communication and the pilot’s service and funding terms. Interest in a partnership does not mean a contract or insurance benefit is in place.
The focused pilot does not include claims processing, an insurer reporting portal, a virtual clinic or a peer community. We can discuss future reporting needs without making them prerequisites for learning from the first families.
