The Parea pilot

Start with what needs support.

A small first group. A shared plan. Care coordination, help from Helen and coaching for the people supporting you. Here is what we are building the pilot around.

Who it is for.

We are planning for young adults 18 to 30 with disabilities or complex conditions who need support moving from pediatric to adult care. Earlier transition planning can begin from age 14.

Our current planning covers New Jersey, Connecticut and New York. If you are a parent, sibling or another supporter, you can contact us about the family’s needs. We will discuss the young adult’s involvement and the appropriate consent before care begins.

What is included.

The pilot is centered on a few connected kinds of help.

  • Care coordination

A named navigator, a shared plan and tracked requests for referrals, refills, equipment, coverage and services.

  • Helen’s AI assistance

Help understanding request updates, preparing for appointments and drafting notes that you choose whether to send.

  • Family support and caregiver coaching

Permission-based care updates and conversations about appointment preparation, changing roles and sharing responsibilities.

How participation begins.

First, tell us who you are and the kind of support you are looking for. A conversation with the team comes before enrollment. Interest does not reserve a place or create a care relationship.

If the pilot is a fit, we will explain the available support, contact arrangements, consent, any costs and what happens next. You and your navigator then agree on the first priorities.

Costs and coverage.

Pilot funding and participation terms are still being finalized. We will explain any cost before you agree to take part. We are not taking payments through the website for the focused pilot.

Having Medicaid, Medicare or another health plan does not by itself mean that Parea is a covered benefit. The team will discuss what is available for your situation; the website does not make a coverage determination.

Keep the first phase focused.

Peer groups, peer mentoring and a caregiver forum are planned for later. The pilot also does not include a virtual clinic, weekly medical visits, prescribing, automated health monitoring or a health-plan reporting portal.

You keep working with your existing doctors. Helen cannot diagnose or recommend treatments, and Parea is not an emergency service.

Learn together.

We will ask whether requests move forward, next steps are easier to understand and families feel more supported. Your feedback will help us decide what to improve before serving more people.

We are also speaking with organizations interested in referring families or helping fund a pilot.
