Who it’s for

A life taking shape. Support that keeps up.

You may be meeting new doctors, planning for school or work, or working out who can help with everyday life. Parea starts with what you want to do and the care that needs to come together around it.

For young adults finding their next step.

Our pilot is being planned for people ages 18 to 30 with disabilities, rare or complex conditions who need help moving from pediatric to adult care. Earlier transition planning can begin from age 14.

You do not need a particular diagnosis, a tidy folder of records or a finished plan to start a conversation. The team will discuss your needs, the pilot’s capacity and whether the available support is a fit.

  • A new care team

You have a referral to an adult specialist, but need help finding out which records are missing and whether an appointment has been booked.

  • Equipment and everyday plans

An equipment request is still waiting on a response, while you are trying to get ready for school, work or a change at home.

  • Paperwork that keeps coming

A renewal or service deadline needs attention, and you want help identifying the right contact and following the next steps.

For the people helping you along the way.

Parents, siblings, partners, friends and other supporters often carry work that no one sees: chasing calls, preparing questions and making sure the next person has the right information.

Parea helps share that coordination work. Caregiver coaching gives families a place to prepare for appointments, talk through changing roles and decide how to share responsibilities. The young adult’s wishes guide who is involved.

How family support works

Different ways to take part.

You can tell us how you prefer to communicate, what helps you understand information and who you want beside you. A supporter can help with the first conversation; using the app independently is not a condition of asking for help.

The app offers text-size, contrast, easy-read and motion settings. If you need an interpreter, communication support or another accommodation, tell the team so we can discuss what can be arranged before you join.

A planned pilot, with a personal conversation first.

Current planning covers New Jersey, Connecticut and New York. We will confirm available support, contact arrangements, consent and any participation costs before enrollment.

Parea’s pilot brings together a navigator, Helen’s AI assistance and family support. You continue to work with your existing doctors and services. An interest message starts a conversation; it does not establish eligibility or enroll you in care.
